You live in Southeast Texas - This includes Jefferson,
Orange, and Hardin Counties, as well as parts of Newton, Chambers, Tyler,
and Jasper Counties. If you live in Newton, Chambers, Tyler, or

You have limited income - To find out if your income qualifies, please
call the "Gift of Life" office at the number below.

You are at least 45 years of age with these exceptions:
- Men who are African American may be as young as 40
- Any man, regardless of race/ethnicity, may be younger than 45
if there is a history of prostate cancer in the family
(father, brother, or son had prostate cancer before age 65)

You do not have private insurance, Medicaid, or Medicare.
Exceptions:
The "Gift of Life" will provide free screenings to men who are
underinsured (those who have a high deductible or a high co-pay.) However,
if you are underinsured, we are happy to provide the free screening,
but if follow-up care is required, you will be financially responsible
for part or all of your care.
Further, we will provide free screenings to men who have Medicare, Part A,
but not Part B (as long as you meet our income limits); however, if
follow-up care is required, you will be financially responsible for
part or all of your care.

You have never been diagnosed with prostate cancer.

You have not had a prostate cancer screening in the past 11 months.
|
Eligible clients with abnormal test results will have access to follow-up medical care.
